Solano Resource Conservation District uses many virtual activities and platforms to support student learning in the online environment. To illustrate these activities to our funders, staff, partners, media contacts, and the general public, we may publish exemplary student stories and work which demonstrate such learning. Please complete the form below to grant permission to Solano Resource Conservation District, its agents, employees, and funding partners to display a student’s photograph, video, voice and/or likeness, as well as any related student work, as well as their first name and city and/or school district, for the primary purposes of reporting, training, evaluating, and marketing any or all programs.
I hereby irrevocably consent to the perpetual unrestricted use of my name, voice and/or likeness by to Solano Resource Conservation District, its representatives and employees in connection with any advertising or promotional activities. I agree that no advertisements or promotional materials need be submitted to me for approval. I further agree that any photographs, pictures, slides, films, audio recordings, videotapes and/or movies (collectively the “Results”) taken or made by Solano Resource Conservation District, its representatives and employees in which I appear, or any reproduction of the same, may be used in any manner or media whatsoever by them, their partner agencies, or any person authorized them. Solano Resource Conservation District, its representatives and employees shall be the owner of the Results, including the copyright or other proprietary rights in the Results. I hereby forever release and discharge Solano Resource Conservation District, its representatives and employees and their licensees, guests, patrons, successors and assigns from any claims, actions, damages or demands whatsoever by reason of use of such Results.
I am the parent, legal guardian, or authorized representative of the student named above. I represent and warrant that I have the full legal authority to enter into this Agreement. I have read the information regarding the display of student work.*
